Columbus Park Nursing and Rehabilitation Center is located on the Westside of Chicago surrounded by beautiful parks and cultural attractions.

Columbus Park is a 216-bed skilled nursing facility licensed by the state of Illinois and approved and certified for Medicare, Mediciaid, Medicaid pending and many other insurance plans.

Columbus Park Nursing and Rehabilitation Center is a 5-story contemporary, residential building offering residents various levels of care options. We provide skilled nursing care, skilled rehabilitation services, short term rehabilitation, Alzheimer's/Dementia Care, Hospice Care, Respite Care and assistance with activities of daily living.

Columbus Park has gained community-wide recognition for its high standards of quality care over 25 years.

We provide and promote daily therapeutic and recreational activities which engage socialization between our residents, families and staff.
