Hospice Care Services

If your loved one has been diagnosed with a terminal illness (6 months or less life expectancy) and is in need of palliative and comfort care, Columbus Park is here to help. We offer Hospice Care services through various local hospice providers. The Hospice providers and our staff work collaboratively in all aspects of the patient’s care to include family, friends and attending physician to provide an individualized plan of care. Hospice Care provides secondary support services including pastoral care, therapy, home health aides, counselors, social services and consulting by the hospice medical director. The purpose of Hospice Care at Columbus Park is to provide comfort, pain and symptom managements as well as emotional support through the last stages of life.

Columbus Park believes it is essential to provide both emotional and physical well-being to all parties involved. Therefore, we pride ourselves in partnering with such services to provide quality care for the terminally ill at our facility. We develop an individualized care plan to meet the needs of your loved one as changes occur.
